
The I-DENTI-FIED System™ is a comprehensive portal which provides access to your medical history when it is needed most. Through the I-DENTI-FIED portal a medical provider can gain access to your entire medical history regardless of where it is located or how many medical records exist.
Using a personalized identification number the provider is able to link you with your medical history and obtain the knowledge needed to reduce the time to treat and support quality care. Also, in an emergency EMT’s are able to access the information to stabilize your condition and communicate with the hospital more effectively when you arrive at the Emergency Room.
The I-DENTI-FIED System consists of two components, a personalized Identification Number and a Care Coordination Portal. Using these two components, along with a universal reader, medical personnel can gain access to your health history.
80% of Americans over the age of 25 have, on average, 3.6 discreet health records; and that average increases with each decade. In a provider-centric industry where the patient has become the workaround, I-DENTI-FIED brings all of the medical records for a patient into one view. The linking solution consists of a Care Coordination Portal and a Unique Patient Identifier embedded in a wallet card that can be read by generally available devices, including smart phones. Using the ID as a key, the portal displays the active Personal Health Record (PHR) and soft-links to other personal health records providing a patient’s medical status, with information such as prescriptions, allergies, emergency contacts and medical conditions.
I-DENTI-FIED supports many different forms of technology to read the identification number and gain the access to the consolidated health history. These include:
- An iPhone or Android using a barcode or QR Code scanning
- A standard RFID Reader
- An Internet web page where the Provider can enter the Identification Number
- A phone call to a call center
So in a time of need, medical personnel, in real-time, can identify a patient using one of our identification methods and a universal reader and through our care coordination portal, they can gain access to the medical history of a patient.
