1. How does I-DENTI-FIED Work?

Members who are I-DENTI-FIED possess an RFID chip containing a 16 digit unique identifier.  Since this identifier is used for a specific purpose, to create a link between the member and their health record, it can be used by a member without fear of identity theft.  The identifier is stored within a PHR, either provided by I-DENTI-FIED or your physician, and then used as a key to unlock your medical history when you need it most.

2. What if I move will my information move with me?

Yes.  Since your PHR is located in a central location access to it can be gained through the Internet wherever you may be located.  Also as you work with new physicians or medical facilities at your new location you can easily print a copy of your PHR and provide it to the caregiver to help start your medical record with their office.

3. Where is the chip located?

Currently the I-DENTI-TAG can be obtained as an RFID Wallet Card or it can be embedded into the gum line of a denture or in a dental prosthetic (crown, bridge).  While it is our suggestion that this form of identification is the best, as it is semi-permanent, we do offer the wallet card option for those who either do not wish this approach or do not possess a denture or dental prosthetic.  Both options offer the same level of medical record access.

4. How big is the I-DENTI-TAG™ for the denture?

The physical size of the tag is 1mm x 5mm which, in relative terms, is smaller than a grain of uncooked rice.

5. What information is stored on the I-DENTI-TAG™?

The I-DENTI-TAG™ only stores a unique 16-digit identification number; it does not store any other type of information.

6. What happens if the medical provider or EMT do not have access to a reader to read the number?

In addition to a specialized reader which will display your medical history once it is read there is a secure website that can be used or an 800 call center to gain access to the same information.

7. Can I block certain information from being displayed?

Yes, each field in the I-DENTI-FIED PHR can be locked and will not display in your emergency PHR.

8. Who updates the information in my I-DENTI-FIED PHR™ record?

You or anyone that you trust can update and add important items to your I-DENTI-FIED PHR™ record at anytime, 24 hours/day, 7 days/week. Use your confidential username and password to access your I-DENTI-FIED PHR™’s secure database to input and save your personal medical history, prescription, allergy, emergency contact and a host of other information.

9. How can I be sure that the information stored in my I-DENTI-FIED PHR™ record will be kept private and secure?

Your I-DENTI-FIED PHR™ record is protected in a highly-secured and encrypted fashion meeting all HIPAA patient confidentiality guidelines. I-DENTI-FIED PHR™ uses multiple forms of security technology to ensure that all of your information is safe at all times. As an additional layer of safety and privacy, I-DENTI-FIED PHR™ does not disclose any database information to any third party, nor does I-DENTI-FIED import or export any information to or from the database.  By participating in the I-DENTI-FIED program you are authorizing emergency physicians and nurses or First Responder personnel access to see your I-DENTI-FIED PHR™ record during an emergency.

10. How do I know that emergency room doctors and nurses will scan my dental prosthetic during my ER visit?

While there are no guarantee’s each emergency department that participates in the I-DENTI-FIED System™ will recognize that a patient has been I-DENTI-FIED.   Each I-DENTI-FIED System™-enabled hospital should scan any unconscious, unresponsive, non-communicative or confused patients when they are taken to an emergency department.  As part of our market growth strategy we also try and educate all of the medical professionals in the area as to the existence of the patients who have been I-DENTI-FIED.  In addition with each dental implant we also provide other forms of identification that you have been I-DENTI-FIED.   The hospitals and First Responder personnel are  trained to look for these other signs that a person has been I-DENTI-FIED; like Windshield stickers, wallet cards, USB devices, stickers on drivers license and cell phone.

11. Is the I-DENTI-TAG™ a transmitting or GPS device?

No, the I-DENTI-TAG™ Transponder is not a transmitter or a GPS tracking device; it uses passive RFID technology which means it emits no energy or signal and only stores a unique 16-digit identification number.

12. How do I know if the I-DENTI-FIED System™ is right for me?

The I-DENTI-FIED System™ can be of benefit to everyone (6 years of age or older). Since you do not know when you are going to have your next emergency it is a great idea to have the I-DENTI-FIED System™ just in case.  The I-DENTI-FIED System™ provides you and your family with peace of mind, and could help emergency room doctors and First Responders treat you quickly, accurately and safely by linking you to your personal health records.

13. Where can I get my I-DENTI-FIED System™?

Dentists and dental lab technicians are being trained every day to place the chip into a denture or dental prosthetic. To find out where the closest licensed dentist please conttact us at info@i-denti-fied.com.  For a RFID wallet card you can call 866-994-3669 to purchase and have a PHR set up for you.

14. Does the I-DENTI-FIED System™ work outside of the U.S.?

The I-DENTI-FIED team is currently focused on building our infrastructure in the United States. While you will not find a healthcare facility outside the U.S. with a reader your PHR  wallet card with your Identification Number can still be used by calling 1-866-994-3669 or via the web at www.identifiedphr.com.

15. Will I-DENTI-TAG™ interfere with other medical equipment in hospitals such as X-RAY machines or MRI Equipment?

No. I-DENTI-TAG™ will not interfere with MRI or X-RAY diagnostic equipment.

16. Is it safe to have an MRI or other diagnostic testing if I have a I-DENTI-TAG™?

Yes. It is safe for you to undergo an MRI, CT and other diagnostic testing.

17. Will I-DENTI-TAG™ cause the security alarm to go off in the airport or in retail stores?

No. I-DENTI-TAG™ does not interfere with any airport or retail security technology.

18. Do Police, Fire and EMT teams have I-DENTI-VIEW™ scanners to use when they respond to an emergency or accident?

The I-DENTI-FIED team is in the process of working with local “first responders” to equip and train them on I-DENTI-FIED System™ and how to use the scanners. However, even if your locality has not received the reader or training they can still access your information by obtaining your Identification number from you, a caregiver or your wallet card and calling 1-866-994-3669 or via the web at www.identifiedphr.com.

19. Is there a certain age limit of individuals that can receive the I-DENTI-TAG™?

No. With the wallet card option a PHR can be created for any individual regardless of age.

20. Will I be notified automatically at the time of renewal of my I-DENTI-FIED PHR™ subscription?

Yes. The I-DENTI-FIED PHR™ database will automatically send you a reminder notification prior to the expiration of your initial I-DENTI-FIED PHR™ service agreement.  Notifications will come in the form of email, regular mail and/or a phone call to ensure that you receive prompt notification about any pending expiration, as well as present you with subscription renewal options.

21. How does a hospital become a participating I-DENTI-FIED System™ provider?

To become listed as a participating I-DENTI-FIED System™ provider, each hospital must sign an agreement stating that they have the I-DENTI-VIEW™ scanner and will scan any unconscious, non-response or non-communicative patients.

22. How much does I-DENTI-FIED System™ cost?

The suggested retail price for the wallet card is $90 which includes the card and 1 year subscription in a PHR and the care coordination portal.  For a denture or dental prosthetic placement a dentist will charge approximately $150.00. This includes the I-DENTI-TAG™, dental placement and the first year of the I-DENTI-FIED PHR™.

23. Will my insurance company pay (discount or reimburse me) for my I-DENTI-FIED System™?

The I-DENTI-FIED team is beginning to work with different insurance carriers across the U.S. to teach them about I-DENTI-FIED System™ and its positive impact on improving patient care, as well as reducing medical risk, inaccurate medical treatment, and unnecessary expenses in our nation’s hospitals. At this time, the cost for I-DENTI-FIED System™ is not yet reimbursed by insurance companies; however I-DENTI-FIED hopes that this will change and it will become a reimbursable expense in the future.

24. Who has access to my I-DENTI-FIED PHR™ record?

Access to your I-DENTI-FIED PHR™ record will only be given to those individuals you designate (usually other family members and/or caregivers, or anyone you trust). In addition, Emergency Room doctors and nurses or First Responders will also be granted “read-only” access to your I-DENTI-FIED PHR™ record during the time of an emergency so that they can help treat you quickly, accurately and safely. Since you maintain your personal health data you have the option to choose how much information you share on a field by field basis.

25. How long does it take from the time that my information is updated to be live in my I-DENTI-FIED PHR™ record?

Your medical information is immediately available in your I-DENTI-FIED PHR™ record each time you make and save an update.

26. My doctor uses electronic health records. Is there any way to link my electronic health records to my I-DENTI-FIED PHR™ record?

Yes, the technologies may be integrated to include a web link from your doctor’s electronic health record (depending on what type of electronic health record system your doctor uses) to your I-DENTI-FIED PHR™ record.  I-DENTI-FIED is working to with EMR vendors to make them aware of the I-DENTI-FIED System and the value of using our Identification for emergency access.

27. Can I change my password at any time? How long does it take?

Yes. Your password can be changed at anytime 24 hours/day, 7 days/week. Once changed by you, the change will take place immediately in the I-DENTI-FIED PHR™ system.
